How to Submit an Image:

Currently, only the partner organizations that have contributed funding for this project can submit photographs for inclusion in the project by using the following forms and sending the photographs and forms to the Wood County District Public Library in Bowling Green, Ohio. Please make prior arrangements before delivery of photographs and accompanying forms by emailing Charlene Avery (averych@oplin.org), Local History Specialist.

Forms:
1. Submission Cover Sheet - (16kb PDF) One form per each shipment from each organization.
2. Photograph Submission Form - (28kb PDF) One form per each photograph.
3. Copyright Permission Request - (140kb PDF) One form per each photograph.
4. Loan for Duplication Agreement - (18kb PDF) One form for each photograph that a partner organization does not own copyright to the image.
